Statement from Flippin' Out:

1. Flippin’ Out Tumbling and Gymnastics is taking all recommended precautions to stop the potential spread of the Coronavirus (COVID-19) in our facility by participating in our programs.

2. The Coronavirus (COVID-19) represents a serious health risk.

3. If at any time you, or someone you or your family members have had direct contact with, shows signs of the Coronavirus (COVID-19) notify Flippin’ Out Tumbling and Gymnastics immediately so that we may take the necessary recommended steps to limit the spread of the Coronavirus (COVID-19).

All Parents are required to adhere to the following guidelines and statements prior to sending their child to enter Flippin' Out Tumbling & Gymnastics for any of our programs.

To the best of my knowledge that neither I nor anyone with whom I reside, including my children, have come in direct contact with anyone during the past 14 days who has:

• Been diagnosed with or currently tested positive for Coronavirus (COVID-19).

• Is awaiting the results of a Coronavirus (COVID-19) active infection test.

• Is under quarantine as recommended by a medical professional or is self-quarantining due to Coronavirus (COVID-19) symptoms.

• Has any of the symptoms of the Coronavirus (COVID-19) as set forth by the Centers for Disease Control (CDC).

• Has had contact with someone who the person believes has or potentially could or might have the Coronavirus (COVID-19).

• Has had contact with someone who had contact with someone diagnosed with the Coronavirus (COVID-19) or who is believed to have the Coronavirus (COVID-19) or symptoms thereof.
